Teammate Spotlight on Summer Schwartz

Summer Schwartz joined Faith Presbyterian Hospice (FPH) in June 2025. In her role as the Admissions Manager, she oversees the hospice admissions process making sure every referral is handled with compassion, accuracy and prompt communication. She also supports the intake team and admission nurses to provide a smooth experience to families transitioning to FPH services. Read about this committed teammate.

  • Professionally, I am passionate about: I am passionate about supporting patients and families during one of the most challenging times in their lives. I care deeply about making sure every detail is handled well so families feel comforted, supported and confident about their care. 
  • Favorite memory/moment to date during your time at FPH: I’ve enjoyed watching our admissions team grow and become more collaborative. We’ve worked hard to strengthen communication between intake and the field, and it’s rewarding to see our work positively impacting families. 
  • What do you think sets FPH apart from other hospice services? What sets FPH apart is the genuine compassion that every teammate shares. We strive to provide excellent clinical care, but also to honor each patient’s life story and make sure families know they’re not alone. 
  • I am most proud of (this can be a personal or professional accomplishment): I am proud of strengthening the intake process, helping our team expand to better support our growing community and maintaining service excellence. 
  • On the weekend, you can find me: You can find me spending time with my family and reading my Bible.
  • My favorite foods/restaurants include: I love dining at The Capital Grille and Chick Fil A, and I always enjoy a good burger.
  • Favorite dish(es) to order: My go-to dish is blackened salmon with steamed broccoli but I also love trying new things. 
  • In my free time, I enjoy (hobbies): My hobbies include cooking, shopping and watching my daughter play volleyball.
  • Share about your family (significant other, siblings, parents, pets, etc. – anyone meaningful in your life, if you have children or pets – feel free to include names, ages/breeds, etc.): I have been married for 12 years and we have a blended family. We have the privilege of raising six children, four girls and two boys: Jade, Brayden, Gianna, Josslynn, Joshua and Bailey.  We also have three grandchildren: Bexlee, Aiden and Olivia. Our two cats, Mino and Suki, complete our family.  
  • How would your friends describe you in three words? My friends would say that I’m smart, strong and funny.
  • What’s something that people might be surprised to learn about you? I used to play in a pool league and won several awards and trophies, including earning the title of 9 Ball Regional Champion for the TriWest county as well as league MVP for 2024!
